Please print this form and turn it in to the office where your child will be attending along with the following documents.

1. Two proofs of residency: current mortgage or lease and one utility (electric, water, sewer, gas) bill.
   *Please note: without 2 proofs of residency, you will not be able to enroll your child(ren).
2. Valid Parent email.
3. Valid Parent ID
4. Valid birth certificate for each student.
5. Up-to-date immunization records for each student.
6. Any court documents in effect for your family.
